• Person-Centered Support: assistance navigating complex access to resources so our members live safer, healthier, more informed lives
  • Warm Houses: engage and connect members to create community within their neighborhoods to prevent or overcome social isolation
  • Learning Social series: present  National Aging Initiatives that inform and engage members in redefining health
  • Healthy Living Sessions: promote self-management of common conditions, i.e. diabetes, depression, chronic pain etc. using Stanford University's certified curriculum. Participation in this support network has resulted in healthier lifestyle choices and fewer hospital admissions.
